This 2-page sheet will provide you with descriptions of services, who to contact for what and the respective contact information for the people you may need to reach. It's a great resource for groups and is color coded to match our forms too!

Community Activities
This is the requisition form that allows you to perform transactions from your CA Budget account. This form is able to be filled in and signed electronically. Using this requisition enables students and advisers to sign and submit electronically. The features of this form work best when opened in Adobe.
Community Activities Requisition Form (Pink) FILL/SIGN CAPABLE
Are you an OPEN student organization in need of a budget packet to apply for a CA Budget for the upcoming budget year of 2024-25? If so, you’re in the right place, but the 2025-2026 Community Activities Budget Packets aren’t available to date. Check back between Nov 2024 - Jan 2025. Feel free to take a look at what last year’s packet looked like by clicking below.
Husky Fund

Here is a PowerPoint presentation that will help provide the fundamentals of what the Husky Fund is, how to access and use the Husky Fund and who to contact for additional resources if needed. This presentation goes through how to complete the Husky Fund requisition for each type of transaction that groups/organizations will be looking to do with their Husky Fund. CGA Services Contract & Blank W-9 Form
This contract and W-9 form is used whenever a group/organization might be paying someone for services rendered, such as performer, official, referee, etc. When using the CGA Service Contract a completed W-9 for the vendor must be completed and accompany the contract, when submitted with the appropriate account requisition (Community Activities or Husky Fund).
This version of the CGA Services Contract is able to be filled in and signed electronically. Using this version enables students, advisers and contractors to sign and submit electronically or to be printed and completed in hard copy.
NOTE- When completing a CGA Services Contract, be sure that the name furnished on the W-9 is the name used on the contract and requisition form (exactly). Along with the service provider's name the group/organization contracting the service should be completing #1-7 before furnishing to the service provider for review and signature.
The blank W-9 form should be given to any service provider to complete and return (based on their tax filing) to the group/organization, in order for them to complete the contract correctly.

CGA Vehicle Request Forms
CGA and the Community Activities Office has a fleet of vehicles that may be reserved by organization/ club members in order to assist with organization/ club activities. Reservations may be made with 30 days prior notice to the date of use (based on availability and established priority list), as long as the group has an approved driver. This section contains both the forms to become an approved driver and the vehicle reservation form.
This form enables drivers 21 years of age or older to apply to become a CGA Approved Driver. Once a driver is approved, they are able to reserve and drive the CGA Vehicles as needed for their organization/ club. If you have questions regarding this form, feel free to call 570-389-5125 for assistance.
This form enables approved drivers to request vehicle reservations with 30 days prior notice to the date of use (based on availability and established priority list).
