Web Portal (Medical Record)

  • The Web Portal is where you will find access to your secure on-campus medical record and is used for completing required medical forms, viewing and printing copies of your billing statements and immunizations and messaging the health center staff.

Having difficulty accessing the Web Portal?

  • Especially MAC users, try using Firefox, Internet Explorer or Google Chrome.
  • Please allow 7 to 10 days after payment of your deposit of acceptance before trying to access your web portal.

Make a Student Health Center Payment

  • Click “Student Health Center Payment”
  • Click “Health Center Payment” product
  • Enter payment amount
  • Click “Add to Cart”
  • Click “Checkout”  (there are no promotional codes)
  • Enter Buyer Information: Student First and Last Name, CU ID#, Date of Birth and Email Address.
  • Click “Continue”
  • Do not register, enter email and Check out as Guest

OR...

  • Enter your email address (for receipt purposes) and select “Checkout as Guest”
  • Enter Payment Method and required Information
  • Click “Continue” (payment can be made with credit/debit card)
  • Review Order (Window will appear - contact information can be edited at this time).
  • Click on “Submit Order”
  • Order Receipt will display and can be printed and an email is generated.
  • Print email for your records.

  • If you are ill and unable to attend your classes, please read and follow the Course Syllabus regarding class absence policy.
  • If a professor requires documentation for your illness absence, call the SHC on the same day you missed your class. We will provide further direction.
  • After an evaluation with SHC medical staff, the student may request a “Verification of Visit”
  • If you are evaluated by a medical provider outside of the SHC, ask for a “doctor’s note” to show to your professors.

Extended Medical Illness (EMI)

  • An EMI is a formal notice sent over CU Succeed by the SHC staff if three (3) or more consecutive days of missed classes are due to an illness or injury.
  • Contact the SHC for more information.

The SHC will not give out any information regarding your medical situation to anyone: family, professors, friends, or even other health care facilities, without your written consent. Professors may not call the SHC to obtain any information on your medical absence. Please refer to the CU Notice of Privacy Practices document in your Web Portal for more information.

Home Medication Management

  • The Student Health Center does not store or administer personal medications.
  • The Student Health Center will not accept mailed packages or medications that belong to students.

Mailing medications to campus

  • Use Fed Ex or UPS
  • Address package as follows:

Special Note

  • Mail Services are closed on weekends and holidays.
  • Refrigeration services are not available.

Storage of Refrigerated Medications (i.e. injectables, insulin)

  • To store refrigerated medication in your residence hall room, special permission is required. Please follow these steps:

Complete these 2 forms:

  1. Disability Services Verification Form
  2. The Special Accommodations form
  3. Once the 2 forms are completed and submitted, Disability Services will contact you with next steps.

Depo-Provera

Depo-Provera injections are administered in the SHC with the following guidelines:

  • You MUST CALL for an appointment and to speak with a nurse — 570-389-4451
  1. If your home medical provider is prescribing you the Depo-Provera, they need to complete and sign the Depo-Provera Order Form.
  2. This order form must be reviewed and approved by our SHC Medical Provider prior to the SHC administering the injection dose.
  3.  Prior to your SHC appointment:
    • Ask your prescribing provider to call the prescription to a local pharmacy.
    • Go and pick up the Depo-Provera from the pharmacy.
    • Bring the medication with you to your SHC appt.

If you are late with your Depo-Provera injection (beyond the recommended dosing schedule on the perpetual calendar):

  • Call the SHC for instructions.
  • We will order a serum pregnancy test the morning of your same-day afternoon appointment with a SHC provider.
  • The Student is responsible for the cost of the serum pregnancy test (test performed at an outside lab)

Birth Control Pill

A prescription for birth control is available after an evaluation by a SHC Nurse Practitioner or Physician. Please call the SHC for an appt.

Emergency Contraception

Emergency contraception (EC) is available at The Student Health Center (SHC) for a fee.
EC is also available OTC in all states with no prescription needed.

EC guidelines
  • EC is most effective if taken within 72 hours of unprotected intercourse, but it may be effective up to 120 hours.
  • Call the SHC to schedule an appointment.
  • Prior to receiving emergency contraception at The Student Health Center:
    • A urine pregnancy test will be performed. Be able to provide a urine specimen.
    • Birth control options and prevention of Sexually Transmitted Diseases (STD's) will be discussed.

Other STI Testing Clinics

SelectPlan for Women

  • Application required.
  • Services include: health exams, routine health screenings and birth control methods. Education, family planning counseling and referrals to other health care providers and social services.
  • Prior to any care rendered in the SHC, the student’s home Medical Provider or Specialist must complete and sign the Medication, Suture Removal, and Wound Care Order Form.
  • The student can drop-off, send through Web Portal or fax the completed and signed form to the SHC.
  • Prior to receiving care in the SHC, the form must be reviewed and approved by a SHC Medical Provider.
  • If approved, the student will be notified to schedule an appointment.
  • If denied, the student will be notified to discuss other care options.

Medication Orders

Student must submit the order form at least one week prior to the anticipated administration of the medication.

Student must bring the medication to the SHC appointment. You may ask your prescribing physician to call the prescription to a Local Pharmacy.

Suture/Staple Removal

If sutures were placed in an Emergency Department (ED), the Medical Director will review the discharge instructions. 

Student must submit the ED discharge instructions or order form at least 4 days prior to the anticipated need for suture removal.

Wound Care

Student must submit the ED discharge instructions or order form at least one week prior to the anticipated need for wound care.

At any time during the treatment, the provider may request the student return to their outside provider for re-evaluation of care.

Need a copy of your medical records or medical information sent to another facility? 

Please read the following information carefully
  • Per PA State Law, medical records are kept for a minimum of seven (7) years from the last encounter date before they are destroyed.
  • Current students may view and print a copy of your immunization record via — Health Center Web Portal.
  • In order to release your medical records, you must complete the Medical Records Release Form (* This form is not intended to be used as a blanket release form).
