Submitting an academic internship form for your student?

Search active internship affiliation agreements and/or clinical affiliation agreements for internship sites BEFORE you submit the internship form.

If there is not an active affiliation agreement on file with the organization or the agreement will expire during the internship, request a new affiliation agreement or have one renewed.
